We take a thorough, integrative, individualized approach to menopause care. That starts with listening to what you're experiencing, defining your goals and preferences, and then building a plan that fits your body. Hormone therapy (HRT, or MHT) is one tool we're very comfortable prescribing when it's appropriate, but it's far from the only tool we can use.
We look at the full picture of your health and walk alongside you every step of the way.
Body Composition Care
Weight changes in midlife are not a character flaw or a sign that you're doing something "wrong." Changes in your hormones can directly affect metabolism, fat distribution, and how your body responds to the same diet and exercise habits that used to work. That's biology, not failure.
In midlife, the optimal body composition strategy often changes. We focus on preserving and building muscle, nourishing your body, supporting nervous system and gut health, and minimizing added stress on your body which can lead to cortisol disruptions over time.
We approach weight care with compassion, no judgment and no one-size-fits-all solutions. We will look at your hormones, thyroid function, metabolic markers, sleep, and stress: all the things that can drive weight changes in women over 40. From there, we build a realistic, sustainable plan that works with your body as it is right now, not as it was ten years ago.

While care is always individualized, treatment may include:

Our goal is to help you manage the changes happening during midlife and menopause proactively, strategically and comprehensively.
This practice is not structured for:
• Emergency, urgent, or same-day care
• One-time prescription requests
• Care for uncontrolled diabetes, extremely high blood pressure, or other medically urgent conditions which would benefit from in-person care and monitoring
Patients are encouraged to maintain a separate primary care provider for acute illnesses, basic preventive screenings, and urgent medical concerns.
SCOPE OF CARE
What we do & don't treat
Every patient starts out with an initial, in-depth medical consultation that is a comprehensive "state-of-the-union" type visit. We will go through everything relevant to your metabolic health and midlife concerns. This includes:
How your weight and body composition have changed over time
What you’ve already tried and how your body responded
Sleep, stress, nutrition, exercise, and recovery
Medical and health history
Any available labs — though having previous labs is not mandatory in order for me to get an understanding of what might be going on with your health
From there, we will walk through a gameplan to address your specific situation.
Some patients decide to continue with ongoing membership care, which includes lab ordering/interpretation, prescriptions, and structured follow-up.
Others take the initial consultation plan and choose to work on it independently or with their existing healthcare team. Either option is totally valid!
If you do decide you want to continue with care, your membership will include ongoing access to Dr. Rowe through regular follow up visits and unlimited secure portal messaging access, all of which are included in your membership.
Membership also includes access to members-only virtual events and coaching sessions, which will be held periodically throughout the year on topics related to women's midlife health.

Membership Fees
Nicole Rowe, MD is a board-certified family physician who focuses specifically on improving women's metabolic health during the second half of life. She combines an evidence-based, integrative health perspective with her background as a women's health coach to help women improve their body composition and overall health.
Dr. Rowe enjoys helping women manage the complex metabolic and hormonal changes that emerge during midlife, including body composition shifts, insulin resistance patterns, sleep disruption, and changes in energy levels. She loves helping women incorporate practical, simple, and effective strategies for improving their health without feeling overwhelmed or burnt out during this incredibly stressful phase of life.
She began her career in primary care, caring for patients across the lifespan. Very quickly, she noticed a recurring pattern: midlife women who were experiencing complex, interconnected metabolic and hormonal changes that were difficult to address within the brief, problem-focused visits typical of traditional primary care.
She started this practice so that she could give patients exactly what they wanted (and what, as a provider, she knows they deserve): deeper and more comprehensive evaluation, more guidance and strategy, and more integrative care than a short primary care visit structure allows for.
Before medical school, Dr. Rowe studied psychology and neuroscience. That background continues to inform her clinical perspective. Sustainable health changes require more than information alone; they involve understanding stress patterns, motivation, behavior change, neurobiology, and the ways these intersect with hormonal and metabolic health.
In addition to her conventional medical training, Dr. Rowe is also a certified physician acupuncturist. Acupuncture training further strengthened her systems-based approach to health, and reinforced the importance of recognizing patterns across multiple body systems. It gave her a broader appreciation and respect for other ways of defining and managing health besides simply taking medications.
In her own life, she prioritizes strength training, fitness, nutrition, and mind-body practices as foundational health habits. These principles shape the strategies she recommends to patients. She has three wild and beautiful young children who keep her laughing daily, and a marathoner husband who has filled their garage with trophies. She loves snowboarding, baking, coffee (always with creamer), and reading.

Q: Do you accept insurance?
A: No. The practice operates on a membership model and does not bill insurance for visits. This allows for longer visits, more individualized care, and greater flexibility in testing and treatment decisions. Patients may still use their insurance for many laboratory tests and prescriptions obtained through traditional pharmacies. When patients prefer to pay directly for labs, we are able to access substantially lower pricing than typical billed insurance rates.
Q: Can I use my HSA/FSA card?
A: Yes. You can use FSA and HSA benefits to pay for care.

A: This practice may not be the best fit if you are primarily seeking quick prescriptions, urgent visits for medical issues, or insurance-based primary care. It is not a good fit for women who have poorly controlled diabetes, poorly controlled high blood pressure, or other serious medical conditions which require urgent stabilization. Q: Why a membership model?
A: Traditional medical care is limited by short appointments, fragmented follow-up, and insurance requirements that prioritize volume over thoughtful care. A membership model allows our practice to operate differently. By only working with a limited number of patients, we spend more time understanding the full picture of your health over time. This allows for more thorough evaluations, more thoughtful treatment plans, and more ongoing guidance than brief, problem-focused visits allow. Membership also allows care to remain independent of insurance restrictions that can limit visit time, testing options, or treatment approaches. The membership model offers a more deliberate, relationship-based approach. That makes care proactive, personalized, and easier to access whenever questions arise.
Q: Do you prescribe hormone therapy?
A: Yes. When clinically appropriate, FDA-approved hormonal therapy including estrogen (estradiol) and progesterone may be prescribed as part of a comprehensive treatment plan. Decisions are guided by evidence-based medical society guidelines, individual risk factors, and overall health goals. Compounded hormones are not currently prescribed, as we prescribe bioidentical hormones using FDA-approved therapies such as transdermal estradiol patches and oral micronized progesterone.
Q: Do you prescribe weight loss medications?
A: Yes, when clinically appropriate, weight loss medications are prescribed as part of a balanced treatment plan, particularly when insulin resistance or metabolic dysfunction is contributing to weight gain. We prescribe weight loss medications alongside lifestyle-based health measures when appropriate, because these medications can lead to muscle loss if used without appropriate nutrition and exercise interventions.
For some patients, medications will be part of a broader treatment plan. For others, meaningful improvement occurs through lifestyle changes and accountability/support, without the need for medication. Treatment decisions are individualized and based on shared decision making and a comprehensive medical evaluation. This is not a one-size-fits-all approach.
Q: Can I use my health insurance for labs and medications?
A: In many cases, yes. Labs are ordered through standard laboratory services when appropriate. Medications can be sent to the pharmacy of your choice and most of them can be processed through your insurance. Please note that weight loss medications are often subject to insurance coverage restrictions.
Q: Is this direct primary care? Can you be my primary care doctor?
A: This is not direct primary care. This practice focuses exclusively on women's metabolic health and midlife body composition management. Patients are encouraged to maintain a separate primary care physician for general medical needs, preventive care, and urgent concerns.
Q: Do you work with women in their 30s or 60s?
A: Yes, when concerns relate to midlife metabolic and body composition goals. However, by law we cannot accept any patients who are enrolled in Medicare or Medicare Advantage plans.

Q: What happens during the initial consultation?
A: The initial consultation is a comprehensive medical and strategy visit. We will go through your health history, symptoms, previous bloodwork, and any metabolic and hormonal changes that are contributing. We will discuss medical and lifestyle factors, and develop a comprehensive strategy for addressing your concerns.
Q: Do I have to join the membership after the initial consultation?
A: There is no obligation to join the membership after the consultation. The initial visit is an opportunity to review your health history and current concerns in detail, develop a thoughtful plan of action, and determine whether the practice is the right fit for ongoing care and implementation of your health strategy. If you decide to continue with membership care, the consultation fee is applied toward your annual membership. If you decide not to join, you will receive a comprehensive summary of recommendations and strategy that you can implement on your own.
Q: How do I know if this is right for me?
A: If you've been told your labs are "normal" but things don't feel right with your body, this is for you. If you're struggling to lose belly fat or perimenopausal weight gain and no one has given you helpful options for treatment, this is for you.
